PREPARING FOR YOUR FIRST VISIT

We appreciate your making an appointment with us and are aware of the stories about doctors being late and appointments taking too long. Unfortunately, some of these stories are true and can often lead to frustration and anger.

We respect your time and try very hard to stay on schedule. We're committed to getting to know you and will spend as much time with you as needed. We ask you to help by arriving for your appointment on time. Remember, too, that it takes about 15 minutes to park and get to the doctor's office, and then about 15 minutes to register and be checked in.

What to bring to your appointment

  1. Medical records (current and past conditions).
  2. X-rays and radiologic films, including reports.
  3. Medicines you are taking.
  4. Names, addresses and phone numbers of the doctor who sent you to see us and the doctor who usually cares for you.
  5. Insurance information.
  6. Worker's Compensation information.
